Health providers strive to ensure well-taught parents ? Health ...

MILWAUKEE, Wis. ? During a home visit with a social worker, in her second trimester of pregnancy, Kalia Yang stares at a chart that shows how many packs of cigarettes an unborn baby is exposed to if the mother smokes: Sixty-seven packs, or 1,350 cigarettes, if she smokes five cigarettes a day in the course of a full-term pregnancy.

Yang is expecting twins in November. She no longer smokes; she quit as soon as she suspected she was pregnant. It?s a big win for the babies and a small victory in Milwaukee?s battle against infant mortality.

Health officials will take it.

In some corners of the city, smoking during pregnancy is seen as a positive that leads to a smaller baby and a smoother delivery. In reality, smoking can cause miscarriage, premature birth and low birth weight. Some women consider breast-feeding uncivilized and unattractive. They don?t grasp its benefits, including possibly providing protection against sudden infant death syndrome.

Knowledge gaps, which exist everywhere, even in a community?s most stable neighborhoods, can be life-threatening if a woman is at risk of having a baby born too early or too small.

Health professionals call this ?health literacy.?

It?s the ability to obtain and understand accurate health information, to manage one?s behavior and to navigate the health care system. It can mean knowing when to see a doctor. It can mean asking the right questions, getting a timely diagnostic test or properly filling out a medical form.

Research shows a strong link between health literacy, health outcomes and medical expenditures.

?You can be very literate and be health illiterate,? said Jackie Tillett, a nurse midwife and director of the Midwifery & Wellness Center at Aurora Sinai Medical Center in Milwaukee.

?Parenting is about being empathetic with your child and caring for your baby. Many people who never graduated from high school are good parents. And I know physicians with advanced degrees who are not good parents.?

A pregnant woman who understands how to care for herself, gets regular prenatal checkups and has support to navigate the health care system has the best chance of delivering a healthy baby.

?It?s not about being stupid or uneducated,? said Nicole Angresano, vice president for community impact at the United Way of Greater Milwaukee, which has joined the city?s fight against infant mortality. ?Information is only as good as it is understandable.?

Yang chose the Wheaton Franciscan-St. Joseph Women?s Outpatient Center for this pregnancy because her sister-in-law had a good experience there when she gave birth. This is Yang?s third pregnancy. The previous two ended in miscarriages, both in the first trimester. Yang blames her miscarriages on smoking, but no one can say for sure why she didn?t carry the babies to term.

Yang first came to the Women?s Outpatient Center because she was bleeding and didn?t know she was pregnant. She was having a miscarriage. She returned several months later to confirm a second pregnancy, but she miscarried again before her first checkup. This pregnancy is the first with regular checkups, frequent ultrasounds and the guidance of a social worker, she said.

Yang, 23, and her boyfriend of six years, Andy Moua, 25, are taking extra precautions this time, trying to do everything right. The couple is in a committed relationship in the Hmong tradition.

For now, the two are focused on learning about pregnancy and healthy babies.

Literacy skills are the strongest predictor of health, more than age, income, employment status, education level, race or ethnicity, according to the Partnership for Clear Health Communication. Older patients, recent immigrants, people with chronic diseases and those living in poverty are especially vulnerable to low health literacy.

One study shows that 40 percent of what shapes a person?s health is related to behavior, a higher proportion than genetics (30 percent), social-environment (20 percent) and access to care (10 percent).

To address behavior, changes are necessary on both sides of the doctor-patient relationship. A patient may not ask questions or share critical information because she doesn?t want to feel dumb. A doctor may assume a message is getting through and not take the time to make sure.

Most brochures that doctors give women to help them better understand pregnancy and possible complications are written at a 10th-grade reading level or higher, said Geoffrey Swain, medical director for the Milwaukee Health Department. But one in five people can?t read above a fifth-grade level, he said.

?Low health literacy makes it harder to get good medical care.?

Meanwhile, a pregnant woman?s friends or family members may pass along misinformation. They may not know doctors now say a baby should sleep on his or her back to help prevent SIDS. Or, one generation may pass a negative stereotype about breast-feeding to the next.

?Health literacy can be overcoming culture,? said Mary Mazul, a nurse midwife at the Women?s Outpatient Center at St. Joseph. ?We all have our own ideas. No matter what culture we come from, what our mothers taught us, not all things are correct.?

Health care providers are homing in on how to better help women have healthy babies.

Obstetrician Tina Mason often writes things down for her patients.

?If you have a patient with a medical problem, the stress of the problem can impact their ability to understand,? said Mason, who also is program director of Aurora Sinai Medical Center?s obstetrics and gynecology department.

?I approach a patient woman to woman, not as a hotshot doctor.?

At the end of appointments, she may ask patients to repeat what she told them so she?s sure they understand. She also coaches medical residents to speak to patients in plain language.

Efforts are under way both locally and nationally to get reader-friendly information into the community.

?We need to reach them in churches, beauty salons and the grocery store, not just the doctor?s office,? said Janice Whitty, chief of obstetrics for Nashville General Hospital at Meharry, who spoke during an Infant Mortality Summit in Milwaukee in May. ?Many women only go to the doctor?s office if they have a problem.?

Social media, such as Twitter and Facebook, are valuable tools to help educate women about healthy pregnancy, she said.

Pregnant women also can get free weekly text messages on their cellphones. The messages in English or Spanish are timed to specific stages of pregnancy. The text4baby service is from the National Healthy Mothers, Healthy Babies Coalition.

Defense Department employee accused of $95,000 bribe (Reuters)

ATLANTA (Reuters) ? A U.S. Defense Department employee based in Afghanistan has been charged with taking a $95,000 bribe from a contractor after federal authorities said they found his backpack stuffed with cash.

Desi Deandre Wade, 39, of Climax, Georgia, was arrested Wednesday in Atlanta, where he had traveled for the Fire Rescue International Conference, officials said on Thursday.

While at the convention, Wade, the Defense Department's chief of fire and emergency services in Kabul, accepted the cash from an Afghan-based company in exchange for the guarantee of a government contract, prosecutors said.

Under the scheme, Wade would provide the company with other bidders' quotes so that it would be the low bidder and get the contract, according to prosecutors.

Federal investigators had received information more than a month ago that Wade was directing contracts to certain bidders for personal gain, authorities said.

"This Department of Defense employee is charged with selling his position of public trust for cash," U.S. Attorney for the Northern District of Georgia Sally Quillian Yates said in statement.

"While he was supposed to be working to support our troops, he was lining his pockets."

Google Buying Motorola Mobility Could Push Android Further (ContributorNetwork)

COMMENTARY | The news that Google is purchasing Motorola Mobility for $12.5 billion could be just the move Google needs to push its Android operating system over the top. The purchase proves the folks at Google are more than serious about pursuing revenue outside of the Internet. Of course, now the search engine-driven giant will be able to push tablets and phones running the Android platform even harder.

The tablet PC market is a jumbled glut of products. While Apple and the iPad continue to dominate, Android powered devices have gained ground. The most recent reports state that Android devices managed to grab 20 percent of the global market share from Apple as far as tablets are concerned, which is impressive. When it comes to smartphones Android has almost 44 percent of the market share. Apparently, if a company sends enough gadgets to market, some sort of success is possible.

Should Google forge ahead with the success of the Android platform they need something that has so far escaped the popular operating system: a marquee product. Sure, a lot of cool devices run a version of Android, and to simplify the development process and the customer process, Google is getting ready to release Ice Cream Sandwich, which is a version of Android that will run on phones and tablets. But unlike Apple with the iPhone and iPad, no device is really synonymous with Android. Perhaps the billion dollar acquisition is part of that strategy.

If Google plans on making their own devices, it would definitely give the company and edge that it has been lacking. Plus, the company also has the cash and advertising resources to get information into the hands of consumers. Surely a major Internet company does not spend almost $13 billion without a long-term plan. In any event, they can simply follow the path that Apple has forged.

The expansion of Google could be a good thing for technology companies and technology in general, but it could also open a door that will be difficult to close. Google carries a lot of weight, and where the company decides to place that emphasis could sway the power of other areas beyond simple Internet searches.

Intellectual property, such as patents, was probably the driving force in Google's acquisition of Motorola Mobility. Whether or not Google uses the company to enhance their market position and continue to play nice with all the companies that license the Android operating system is something every fan will be keeping an eye on.

KANSAS CITY, Mo. - Raheem Morris stood at the podium deep inside Arrowhead Stadium, looked straight into the camera and let a slight smile creep across his face.

The third-year head coach was asked whether he could find something wrong with the performance that his Tampa Bay Buccaneers put in during their preseason opener Friday night, a 25-0 blitzing on the road against the defending AFC West champion Kansas City Chiefs.

Morris resigned himself to the fact he couldn't, at least not yet.

"But I got to watch the tape," he added. "I'll find something."

Josh Freeman put points on the board three of the four series he played, finishing 9 of 13 for 73 yards with a touchdown rushing. Backup quarterback Josh Johnson was just as efficient, going 7 of 12 for 108 yards and a touchdown while also running for 57 yards.

The Tampa Bay offense piled up 353 yards, while a young and rapidly improving defense held the explosive Kansas City offense to just 137 ? 86 on the ground and 51 through the air.

"It's our first chance, you know?" said veteran Tampa Bay cornerback Ronde Barber. "You get two weeks of training camp, you really don't know what you got, so this is a good first game."

It figured to be a sloppy game after the NFL lockout wiped away the offseason. Teams have only been together for about a dozen practices, and veteran free agents couldn't even join their teammates until last Thursday because of new language in the collective bargaining agreement.

Kansas City had barely worked out in pads, and coach Todd Haley decided not to scrimmage during camp, wary that too much hitting too early might result in unnecessary injuries.

It wasn't much surprise the Chiefs looked unprepared.

Quinten Lawrence made a poor decision to bring the opening kickoff out of the endzone, getting wrestled down at the 8. The Chiefs went three-and-out on their first possession, fumbled on their next two, and watched backup quarterback Tyler Palko get sacked for a safety on another.

"Turning the ball over was a problem, not protecting the quarterback's a problem. We've got a lot of work to do," Haley said. "We knew we had a lot of work to do coming into this game."

Freeman needed only two plays to turn the first fumble into a touchdown, then led Tampa Bay on an eight-play drive that Connor Barth capped with a 25-yard field goal. Barth added another field goal later in the half before Palko's safety made it 15-0 late in the second quarter.

Things didn't improve for Kansas City in the second half.

Tampa Bay marched right down field against the Chiefs' backups, putting together a nine-play, 55-yard drive that resulted in a 3-yard pass from Josh Johnson to Michael Spurlock and a 22-0 lead.

Backup kicker Jacob Rogers added a 46-yard field goal early in the fourth.

Freeman is trying to build on a breakout season in which he threw for 3,451 yards and 25 touchdowns. The tall, strong-armed quarterback has looked sharp all camp, and he certainly carried the momentum right into his first preseason game. Freeman checked off when warranted, eluded a feeble Kansas City pass rush when necessary, and deftly managed the offense when he was in the game.

Morris planned to play his starters about 20 plays, so Freeman was done early in the second quarter. Johnson took over and kept the offense moving down field.

"They throw a bad ball, they're down on themselves," said backup receiver Dezmon Briscoe, who caught four passes for 60 yards from the two quarterbacks. "But they rarely make bad mistakes."

That sure wasn't the case for Kansas City.

Not that the starters had much chance.

Cassel didn't throw a pass. Running back Jamaal Charles didn't have a carry. Wide receiver Dwayne Bowe didn't make a catch ? all for the most part by design.

Defensive end Tamba Hali, who signed to a five-year, $60 million deal in training camp, looked awfully good standing on the sideline. Receiver Steve Breaston, cornerback Brandon Flowers, defensive end Glenn Dorsey, tight end Tony Moeaki and linebacker Derrick Johnson also didn't play.

Those who did left Haley wanting more.

"I don't think you ever see that coming," Haley said. "You don't want to do that, especially on your home turf. But that being said, that's the first one, not the last one. What's important is that our guys recognized that they've got a lot of work ahead of them and a short time to do it."

Dow soars 423 points on economic news

Stocks rose following the release of a better-than-expected jobless claims report on Thursday morning. The Nasdaq was up 111 points at the close, and the S&P 500 was up 51.

Stocks are rising at the close of trading after investors latched onto some small signs that the economy might not be headed into another recession.

Fewer Americans joined the unemployment line last week, and a technology bellwether said revenue could grow faster this quarter than analysts expected. The news is pushing down prices on long-term Treasurys down, and gold is down from its record high.

The Dow Jones industrial average is up 423 points Thursday, or 3.9 percent, to 11,143. It's the first time the Dow has ever had four straight 400-point days.

The S&P 500 is up 51, or 4.6 percent, to 1,173. The Nasdaq is up 111, or 4.7 percent, to 2,493. All three major U.S. stock indexes are down at least 1.6 percent for the week.

In patients with acute myocardial infarction (heart attack), blood loss from greater use of phlebotomy (blood drawn for diagnostic testing) appears to be independently associated with the development of hospital-acquired anemia (HAA), according to a report published Online First by Archives of Internal Medicine, one of the JAMA/Archives journals. The article is part of the journal's Less Is More series.

    Anemia (low red blood cell count or low hemoglobin level) is associated with greater mortality and worse health status in patients with AMI, whether the condition is chronic (present at hospital admission) or hospital-acquired, according to background information in the article. However, HAA may be preventable if strategies to reduce blood loss in high-risk patients are implemented. Among the factors that may be associated with HAA is blood loss from diagnostic phlebotomy, which has been associated in other patient populations with in-hospital declines in hemoglobin level and need for blood transfusion. "Blood loss from phlebotomy could be an actionable target for intervention," write the authors.

    Adam C. Salisbury, M.D., M.Sc., from Saint Luke's Mid America Heart and Vascular Institute, Kansas City, Mo., and colleagues analyzed data from the Cerner Corp.'s Health Facts database. Records selected from January 2000 through December 2008 included 17,676 patients who were admitted with AMI and no anemia from 57 hospitals. To measure blood loss, the researchers identified all phlebotomy events in patient records. They determined, by the laboratory tests ordered, which type of hematology tubes were used and the blood volume each held. Then, for every patient, the researchers multiplied these blood volumes by the number of tubes of each type that were collected during hospitalization. They also calculated the mean (average) blood drawn for every 24 hours of hospitalization and the mean phlebotomy volumes for each of the first 10 days of hospitalization.

    About 20 percent of patients (n = 3,551) developed moderate to severe HAA. Estimates of the mean blood loss from phlebotomy for these patients (173.8 milliliters [mL]) were almost 100 mL higher than estimated blood loss in patients who did not develop moderate to severe HAA (83.5 mL) over the course of the hospitalization. The volume of diagnostic blood drawn was associated with developing moderate to severe HAA. The relative risk for developing HAA increased by 18 percent for each 50 mL of blood drawn; the relationship persisted when researchers adjusted the data for site and potential confounders. The average volume of blood drawn varied significantly across hospitals.

    "In conclusion, blood loss from phlebotomy is substantial in patients with AMI, varies across hospitals, and is independently associated with the development of HAA," write the authors. "Studies are needed to test whether strategies that limit both the number of blood draws and the volume of blood removed for diagnostic testing can prevent HAA and improve clinical outcomes in patients with AMI."

    Commentary: Hazards of Hospitalization -- More Than Just 'Never Events'

    An accompanying commentary by Stephanie Rennke, M.D., and Margaret C. Fang, M.D., M.P.H., both from the University of California, San Francisco, evaluated the study by Salisbury and colleagues in the larger context of patient safety in hospitals. "Of particular interest was the finding that the mean phlebotomy volume in patients varied widely across individual hospitals, suggesting that some blood tests may have simply been 'routine' and implying that reduction in the variability of care could potentially lead to reductions in HAA," Rennke and Fang write.

    They cite increased attention to patient safety since the publication of an Institute of Medicine report on the topic in 1999. This focus, Rennke and Fang explain, "has led hospitals to test and implement system-wide interventions, some of which have been found to be effective in reducing the rates of hospital-acquired complications." As an example, they point to a Michigan Health and Hospital Association Keystone program which appeared to reduce rates of hospital-acquired infections and subsequent hospital deaths.

    "With the increasing evidence that health care system interventions can reduce or prevent many hospital-acquired complications, efforts to implement effective strategies to make medical care safer and more effective are crucial," write the authors. "As Salisbury et al highlight in their study, HAA could potentially be considered a hazard of hospitalization. Investigations on how to modify this risk (e.g., through reducing unnecessary phlebotomy or reducing the volume of blood obtained during a hospitalization) could provide important insights into how to reduce anemia in the hospital and improve the value and appropriateness of care."

    AP IMPACT: Japan ignored own radiation forecasts (AP)

    NAMIE, Japan ? Japan's system to forecast radiation threats was working from the moment its nuclear crisis began. As officials planned a venting operation certain to release radioactivity into the air, the system predicted Karino Elementary School would be directly in the path of the plume emerging from the tsunami-hit Fukushima Dai-Ichi nuclear plant.

    But the prediction helped no one. Nobody acted on it.

    The school, just over six miles (10 kilometers) from the plant, was not immediately cleared out. Quite the opposite. It was turned into a temporary evacuation center.

    Reports from the forecast system were sent to Japan's nuclear safety agency, but the flow of data stopped there. Prime Minister Naoto Kan and others involved in declaring evacuation areas never saw the reports, and neither did local authorities. So thousands of people stayed for days in areas that the system had identified as high-risk, an Associated Press investigation has found.

    At Karino Elementary in the town of Namie, about 400 students, teachers, parents and others gathered in the playground at the height of the nuclear crisis stemming from the March 11 earthquake and tsunami. Many ate rice balls and cooked in the open air.

    They were never informed of the predictions that they were at risk. In an interview with the AP, Namie's mayor said it took more than 24 hours for him to realize ? from watching TV ? that the evacuees were in danger. He sent buses to move some of them out. But, unaware of the risks, they were taken to another part of town also forecast to be in the plume's path. Most were left to fend for themselves.

    "When I think about it now, I am outraged," Principal Hidenori Arakawa said. "Our lives were put at risk."

    Documents obtained by the AP, interviews with key officials and a review of other newly released documents and parliamentary transcripts indicate that the government's use of the forecast data was hamstrung by communication breakdowns and a lack of even a basic understanding of the system at the highest levels.

    It's unclear how much radiation people might have been exposed to by staying in areas in the path of the radioactive plume, let alone whether any might suffer health problems from the exposure. It could be difficult to ever prove a connection: Health officials say they have no plans to prioritize radiation tests of those who were at the school.

    But the breakdown may hold lessons for other countries with nuclear power plants because similar warning systems are used around the world. This was their first test in a major crisis.

    The Japanese network ? built in 1986 at a cost of $140 million (11 billion yen) ? is known as SPEEDI, short for the System for Prediction of Environment Emergency Dose Information. It has radiation monitoring posts nationwide and has been tested in a number of drills, including one the prime minister led for the Hamaoka nuclear facility just last year.

    Even so, according to the prime minister's office, Kan and his top advisers never asked for or received the data. Despite taking part in the Hamaoka drill, Kan admitted he didn't understand how SPEEDI worked or how valuable the data was.

    "I had no idea what sort of information was available," he told Parliament on June 17. "I didn't know anything about it then, and there was no way I could make a judgment."

    In two post-crisis assessments, a report to the International Atomic Energy Agency and an annual white paper on science and technology, his government has said the network "failed to perform its intended function."

    A senior member of Kan's crisis team, Nuclear Safety Commission chief Haruki Madarame, went so far as to say the SPEEDI data was no better than "a mere weather report."

    He said the predictions were of no value because they lacked accurate radiation readings. Some of the system's monitoring capabilities were compromised by the tsunami and ensuing power outages, and the utility that runs the Fukushima plant, TEPCO, did not provide readings of its own.

    But SPEEDI officials say Madarame's position reflects a fundamental misunderstanding of what their system is designed to do.

    When the amount of radioactivity that has been leaked is known, that is entered into its system, along with weather and terrain data, and a hazard map is generated. If the amount is not known ? as was the case with Fukushima ? a standard and relatively low value of one becquerel is used.

    While that won't show the actual radiation risk, it will show the general pattern and direction of the plume. Then when the size of the leak becomes known, the map can be updated. If the actual leak turns out to be 100 becquerels, for example, the results would be multiplied by 100.

    That technique allowed SPEEDI to produce reports hours before officials began venting disabled reactors ? when there would have been less radiation to measure outside the nuclear plant even if the system's monitoring equipment had been working perfectly.

    In the Fukushima case, later data proved the forecasts to be highly accurate. Most of Namie, for example, has since been declared too dangerous for habitation.

    "We are offended by allegations that SPEEDI failed to function the way it was supposed to," Akira Tsubosaka, a senior official in charge of operations, told the AP. "SPEEDI was not used to determine evacuation zones. It should have been."

    SPEEDI, run by the education and science ministry, provides its data to other government agencies such as the nuclear safety agency for passage up the chain and then dissemination to local authorities.

    Officials won't say why that didn't happen, sticking to their position that the data was useless anyway.

    But the government response has been sharply criticized by one of Kan's top science advisers, who later quit in protest, according to a confidential report to the prime minister that was obtained by the AP.

    "The SPEEDI radiation forecasts were not properly utilized and a situation was invited in which residents were made vulnerable to more exposure than necessary," Toshiso Kosako, also a professor at the University of Tokyo, wrote in late April.

    Ironically, low-level officials were quick to seek the SPEEDI data.

    Bureaucrats familiar with SPEEDI commissioned at least 18 tailor-made forecasts in the first 24 hours, as the government was pushing TEPCO to open vents to avert an explosion.

    The venting would release radioactive substances into the air. So, according to documents obtained by the AP, the forecasts included several to gauge that danger.

    One issued at 3:53 a.m. ? about 13 hours after the crisis began ? predicted the plume would drift across Namie and several other towns.

    The forecasts were relayed to the Nuclear and Industrial Safety Agency but they did not reach decision-makers.

    In Japan, the legal responsibility for setting evacuation zones falls on the central government and the prime minister. Local officials then are tasked with implementing the orders.

    Instead of following the patterns of radioactive dispersion suggested by SPEEDI, the central government simply set up a six-mile (10-kilometer) evacuation zone around the plant. That did not include a broad swath of land that SPEEDI predicted would be affected.

    The mayors of two towns that have since been almost completely evacuated told the AP that the government did not inform them of even that decision ? let alone provide SPEEDI data ? so they had to act on their own. They said they were unable to assess the risks adequately because they were not privy to the SPEEDI reports.

    "We got nothing until more than a week later," said Katsunobu Sakurai, the mayor of Minami-Soma. "People were unnecessarily exposed to possible dangers. We believe the central government must come clean on this."

    "The first I heard of the 10-kilometer zone was when I saw the news on TV," said Namie Mayor Tamotsu Baba. Namie's municipal government has since been evacuated to Nihonmatsu, a city 30 miles (50 kilometers) from the plant.

    Because Karino Elementary sits just outside the six-mile (10-kilometer) evacuation zone, it was used as a gathering area for evacuees.

    Later in the day, at the mayor's order, some evacuees were taken by bus to another part of Namie called Tsushima, which SPEEDI data suggested was also dangerous. Others, including Principal Arakawa and his family, went in the same direction by car.

    Masako Mori, a senior opposition member of Parliament from Fukushima, told the AP that two alternative routes would have led away from the areas identified as high-risk by SPEEDI. The third ? to Tsushima ? led along the plume's expected path.

    "We didn't have any information. But it turns out we were taking the most dangerous route," Arakawa said. "None of us knew."

    Mori said SPEEDI data should have been used to get people out of the area much faster.

    The evacuees at shelters in the Tsushima district ? including about 8,000 residents of Namie ? were not told to move farther away until March 16, five days into the crisis.

    Mori, who also is a trial lawyer, raised the possibility of lawsuits against the government.

    "The government unnecessarily exposed people to radiation, failing to observe its legal obligation to protect the citizens," she said. "It could be held responsible for compensation for the possible damage caused by its errors."

    Exposure to radiation can lead to a variety of cancers ? as it did in Chernobyl. Babies, children and pregnant women are at the highest risk.

    Mori, along with Namie's mayor and the school principal, are seeking full-body radiation tests for all children who were at the school. The tests measure internal exposure such as inhaled radioactive particles and could be key to understanding the health impact. But Fukushima health officials say they have no particular plan to test the Karino evacuees, because they don't have the resources and are instead focusing on groups, such as pregnant women, from the general area.

    Thousands across the region have requested full-body tests, and only 340 have gotten them so far. None had dangerously high levels of contamination, though that does not rule out future health problems.

    The Fukushima health office said it is looking into ways to speed up the process.
